For my long term CCSF patients, I recommend you contact your adjustor or check with your colleagues for assignment of a new Primary treater. Send us a written request (or 1 email) of where you want your records sent to. That will take 5-7 days.

We are available to schedule appointments and answer our patients' questions Monday through Friday from 9AM-5PM. We are closed for lunch from 12-1:15 daily. We require 24 hour notice on any appointment cancellations.

Current insurance card/referral, if needed
WC Information - adjustor name, Claim #, Company name and address
Current list of medications and allergies
Past X-rays, MRI, operative reports, bone scans or medical records
Pair of shorts if problem involves the knee or a tank top if being seen for your shoulder
