Office Hours

We are available to schedule appointments and answer our patients' questions Monday through Friday from 9AM-5PM. We are closed for lunch from 12-1:15 daily. We see patients from 8-12:30 on Tuesday and Thursday, from 3-5PM on Monday and from 1-5 PM on Wednesday. Dr Anderson is in surgery on Mondays and Thursdays. We require 24 hour notice on any appointment cancellations.
 
Patients Should Bring

  • Current insurance card/ referral if needed
  • WC Information- adjustor name, Claim #, Company name and address
  • Current list of medications
  • Past X-rays, MRI, operative reports or bone scans
  • Pair of shorts if problem involves the knee or a tank top if being seen for your shoulder

Insurance Plans

Dr Anderson is contracted with most PPO's. She is not contracted with Blue Cross PPO and Blue Shield PPO. We recommend that you check with your current insurance to verify that Dr Anderson is a participating provider. You will be required to pay your copayment at the time of your visit. If we do not participate with your health plan, payment at the time of service will be required. We are happy to bill your insurance carrier as a courtesy and assist with the paperwork.

If you belong to an HMO, you will also be required to present your referral at the time of your appointment or you may need to have your appointment rescheduled. Unfortunately our office cannot obtain this for you. You also will be required to pay your co-payment at the time of service.
 
Workers' Compensation/Industrial Injuries

We are required to obtain an authorization from the Worker's Compensation carrier before seeing you in the office. If you have been hurt on the job, be sure your employer has been notified and you have received permission to be seen in our office. If your treatment is denied as being non work-related, you are financially responsible for all charges incurred.
 
Third-Party Claims

If you are involved in a third party litigation situation, or have been involved in an automobile accident, you will still be required to pay for services rendered at the time of your visit. We cannot bill for your care since these claims often take many months or years to resolve. The only exception is if the insurance company gives us a guarantee of payment. It is the patient's responsibility to collect from third-party payers or negotiate a settlement. If this situation applies to you, please contact our office in advance of your appointment.
 
Disability Forms

Our staff is happy to fill out state disability forms at no charge. All other disability forms must be accompanied by a fee of $20 per form prior to completion. We require five working days for completion. There is an additional $10 fee if the form needs to be rushed or completed with 24 hours.
 
Medical Records
Our office keeps electronic copies of your medical records. We will be able to provide a one time copy of your medical records provided that the records are fewer than 50 pages. If the records exceed 50 pages, there will be a charge of $30 each time records are requested to cover the cost of time and materials. There is no charge if you wish the records to be faxed. We require five working days for completion. There is an additional $10 fee if the records need to be rushed or completed with 24 hours.

If your records are stored at our off-site storage facility (patients who had no appointment after 2004) there is an additional $20 fee and we are unable to rush this request. All records after 2001 have been destroyed.

We do not store MRI or X-ray films. Please contact the facility directly.

To protect your privacy, no records can be released without a written consent. Please complete the below medical release and forward this to our office.

Pre-Surgery Preparation

  • Nothing to eat or drink 8 hours prior to surgery or surgery may be cancelled for that day.
  • Wear loose fitting clothing
  • Arrive at least two hours before your scheduled surgery time
  • Fill your pain medication prescription before your operation
  • Make sure there's someone to take you home.
  • Reread your pre and post operative surgery instructions the day before surgery.
  • If you have a cold, flu, or fever, contact Dr Anderson the day before surgery since you may need to be rescheduled
 
After Hour Emergency Procedures

If there is an urgent need or a postoperative question after office hours call 415-923-3029 to have Dr Anderson paged. If you feel your problem is an emergency either call 911 or proceed to the nearest emergency room. Medications are not refilled after hours or on weekends. If you are canceling or changing an appointment or have other non-urgent questions please leave a message on our voice mail.
